Contact and handling fee

Persons requesting the disclosure or modification of Personal Information, or the termination of its use due to the violation by IMS of Article 16 or 17 of the Personal Information Protection Act, should send the request by postal mail to the following address.

Handling fee:
Please enclose 1,000 yen in postage stamps as the handling fee.

Proof of identity (of requestor or agent)
Please enclose a copy of one of the following types of documentation indicating your identity.
1) Driver's license; 2) Health insurance card; 3) Residence certificate; 4) Passport; 5) Alien registration card
If application is made through an agent, please enclose a copy of one of the above documents along with a written Power of Attorney designating the agent (if a statutory agent acting on behalf of a minor, enclose a copy of a family register abstract or residence card indicating the relationship). If the agent is an attorney, please indicate the attorney registration number.
